Effective February 1, 2011, all Fringe Benefit Contributions and Contribution Forms should be remitted to the following address: They should NO LONGER be mailed to the Harris Bank, P.O. Box in Chicago, Illinois, the address that is currently listed on your reporting form. To read the informational letter sent to contractors in January 2011, please click here.
